Transitioning Services
- The purpose is to provide support to families/natural supports during the transition from Children’s to adult services
- The individual must be in existing services offered by Taber Special Needs Society
- Taber Special Needs Society will act as a “hands on resource for Families”
- Assist families with agency connections (AISH, PDD, Public Guardian…)
- Assist families with the planning process
